Requirements for Company participation in the Summer Youth Employment Program:
Organization needs to enter into an Agreement with WorkForce One Employment Solutions.
Organization needs to print, sign and submit the Agreement to WorkForce One.
Jobs requiring lifting by Youths will not be considered, for liability reasons.
Employers must comply with all applicable components of the Child Labor Laws.
Employers must FAX your organization's
Certificate of Insurance
to (954) 202-3619.
This page (Company Information) must be entered ONLY ONCE for your
entire organization

Instructions to complete your Job Order request
Please complete the information below to request youth to work for your organization during the
Summer Youth Employment Program.
Section One
, called “
Company Information
”.
○ It should be completed
ONLY ONCE
per Organization/Company. If you or anyone in your organization have already completed this section, please log in as a Returning Employer at the top of this page.
Section Two
, called “
Worksite Information
”.
○ An organization with multiple facilities or locations will then have to complete multiple
“Worksite Information”
form. (i.e.: Broward County Libraries - Main Library, North Lauderdale Library, Carver Ranches Branch, etc.)
Section Three

Job Positions at this Worksite
”.
○ One for each location or worksite, and/or multiple
“Job Positions at this Worksite”
forms.
After you complete
Section One
and again after you complete
Section Three
you will receive an
ID Number
.
○ You will need this ID number in order to log in to add Worksites or Job Titles at a later date.
○ You may then return to Section One, click “Log In” at the top of the page, and continue to add more Worksites (returning to Section Two) and/or more Job Positions (returning to Section Three), to complete your
JOB ORDER
.
If you are not ready to complete a JOB ORDER with at least one Worksite and one Job Position information (Sections Two & Three), click the "Cancel" button now.
HELPFUL HINTS
Use your Employer ID and Tax ID to log in again if you need to add more Worksites or Job Positions at a later date.
To make changes/corrections to information previously submitted in a JOB ORDER, you will need to contact WorkForce One at (954) 202-3830 for assistance.
Exactly ONE "Company Information" form (Section One) will be completed for your entire organization. Your ID Number allowing you to log in later is based on that one entry. All of your Worksites, Jobs and Youth assigned are associated to that ONE ID Number. One Worksite Information form (Section Two) will be completed for EACH DIFFERENT WORK LOCATION. One Job Position Information form (Section Three) will be completed for EACH Job Position at a work location or "Worksite". You can request as many Youth for a Job Position as you like, but the Job Position Information form is filled out only once for each type of job. There is a box on the form to enter the
number
of Youth you are requesting for that Job Position.

(If you are not ready to complete Section Two and Section Three, but want to submit Section One and add Worksites later, click “Continue” and write down the Employer ID you will be given on the next page (Section Two).
Give that ID Number to all other Worksites that will be included in this Job Order (See instructions above).
